Description
Essential Duties and Responsibilities:
- Provide support to individuals in 1:1 and group settings as defined in their Individual Support Plan (ISP) and Day Habilitation Service Plan (DHSP).
- Record accurate data for established goals, objectives, and behavior plans; prepare progress notes and other required documentation using professional standards.
- Collaborate with Case Managers, clinicians, and paraprofessionals to develop and implement programs that enhance skills in self-care, communication, self-direction, self-preservation, and activities of daily living.
- Assist in developing and implementing behavioral and therapeutic intervention strategies as outlined by Occupational Therapy, Physical Therapy, Speech Therapy, Nursing, and Behavioral Services.
- Support Positive Behavior Support (PBS) initiatives and complete Quality Behavior Curriculum requirements.
- Implement behavior plans and interventions as assigned, including physical restraint when required for safety and in compliance with Safety Care training.
- Develop curriculum, activities, and modules to enhance program quality and member engagement.
- Participate in interdisciplinary team meetings and provide input during reviews and planning sessions.
- Serve as a positive role model for individuals, establishing consistent behavioral expectations in accordance with agency policies.
- Provide coaching and support to new or less experienced staff as directed.
- Maintain regular communication with Case Managers and supervisors regarding member progress and concerns.
- Attend all required meetings, trainings, and certifications (including Safety Care).
- Ensure program routines, schedules, and safety standards are followed at all times.
- Assist and support individuals with physical and dietary care needs, including physical or mechanical transfers, assistance with feeding, or delegated nursing activities (e.g., administration of thickening agents or special diets).
- Provide transportation for community and program activities as needed.
- Engage in remote or virtual services following a preplanned curriculum; assist in developing virtual materials as appropriate.
- Follow all current OPTS/Day Hab guidelines based on DDS/MassHealth regulations, including daily health screening and PPE compliance.
- Serve as a liaison between RTR and the community, representing the agency with professionalism, empathy, and a proactive attitude.
- Adhere to all RTR policies, procedures, and safety standards.
- Perform additional duties or substitute for others as assigned.
- Provide all services in alignment with RTR’s mission, philosophy, and core values.
Requirements
- High School Diploma or equivalent.
- Valid Driver’s License.
- Basic knowledge of Microsoft Word and the internet.
- Successful completion of all state and agency mandated trainings.
- Must be willing to authorize a Criminal History Background Check (CORI) and release of information concerning allegations from Disabled Persons Protection Commission (DPPC), Department of Developmental Services (DDS), and other agencies.

- Tuberculosis screening within the previous 12 months.
LANGUAGE SKILLS
Must be fluent in English and have the ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to communicate effectively with individuals under supervision, employees of the organization, families, and members of the community.
