OptumMD is a concierge internal medicine practice redefining primary care through deeply personalized, prevention-focused care. Rooted in a functional approach to medicine, nutrition, and longevity, we consider the full picture of each patient’s lifestyle, environment, and biology. With a Registered Dietitian and Physical Therapist already on the care team, we’re expanding our offerings by adding a Health Coach to support patients in optimizing their habits and achieving long-term wellness with a goal of enhancing healthspan and lifespan.
Position Overview:
We are seeking a personable, skilled Health Coach who is passionate about helping patients Implement meaningful, lasting changes in the areas of physical activity, sleep, stress management, and behavior change. This role is designed to complement medical and nutrition services already in place, and to help patients build sustainable, lifestyle habits through a functional lens.
The ideal candidate will have strong motivational interviewing (MI) skills, be highly empathetic, and thrive in a concierge care environment where trust, time, and transformation are valued over volume.

Requirements
Responsibilities:
- Deliver 1:1 coaching sessions aligned with functional medicine care plans inclusive of the following:
- Physical activity and movement routines
- Sleep hygiene and circadian rhythm alignment
- Stress resilience strategies (mindfulness, relaxation, time management)
- Habit-building and behavior change using evidence-based frameworks
- Apply motivational interviewing techniques to help patients uncover personal motivations, navigate ambivalence, and build confidence in making change
- Collaborate closely with the physician and Registered Dietitian to reinforce diagnostic findings and treatment plans along with patient's overall lifestyle goals
- Track progress, identify barriers, and adjust coaching strategies accordingly
- Offer group education, workshops, or wellness resources as appropriate
- Document sessions accurately and confidentially per HIPAA and practice protocols
Preferred Candidate Qualities:
- Excellent communication and motivational interviewing skills
- Ability to guide conversations with empathy, structure, and goal-orientation
- Passion for functional and whole-person health
- Comfortable working within a small, patient-centered concierge practice
- Highly organized, dependable, and proactive
Qualifications:
- Bachelor’s degree in Health Promotion, Psychology, Kinesiology, Public Health, or related field (Master’s preferred)
- Preferred Health Coaching certification (NBHWC, FMCHC, ACE, Duke, or similar required)
- Proven experience using motivational interviewing in practice
- Experience working with high-performing or high-expectation patients
- 1–2 years of professional health coaching or behavior change support experience
- CPR/AED certified (or willing to obtain)
- Bonus: Experience with sleep coaching, personal training, stress management, or fitness instruction
- Bonus: Familiarity with GI health, hormonal health, metabolic health, stress physiology, or autoimmune conditions
