Job Summary
Phoenix Home Care and Hospice is actively hiring compassionate and skilled Licensed Practical Nurses (LPNs) and Registered Nurses (RNs) to provide one-on-one private duty nursing for children and young adults in their homes. We are currently offering Full-Time, Part-Time, and PRN positions, allowing you to choose the schedule that best fits your lifestyle.

Weeknight shifts: +$3 per hour
Weekend days: +$4 per hour

Responsibilities
Provide skilled, compassionate one-on-one nursing care in the home for pediatric and young adult clients
Follow individualized care plans and deliver treatments as prescribed
Monitor, assess, and document patient condition accurately and communicate updates to the care team
Educate and support families to ensure continuity and quality of care
Build trusting, meaningful relationships with clients and families
Requirements
Active and current LPN or RN license
Pediatric or home health experience preferred but not required; training is provided
Strong commitment to patient-centered care
Excellent communication and interpersonal skills
Ability to pass background checks and meet all regulatory compliance requirements
