About this role:
The Community Based Day Services (CBDS) Direct Support Professional facilitates community supports for individuals with developmental disabilities, utilizing a team-based approach. The CBDS DSP promotes individual growth in the areas of choice and decision-making, community membership, household management skills, relationships and personal accomplishments.
Schedule: Monday through Friday, 8 am - 4 pm
Location: Sandwich, MA
Compensation: $21-$24 / based on experience
As a KDC Case Manager, you'll get to:
- Support individuals with disabilities in a community setting.
- Promote individual choice and self-expression based upon quality service principles.
- Facilitate assessment and development of individual treatment objectives by implementing and documenting individual programs
- Ensure completion and maintenance of documentation as required by KDC and all applicable funding agencies.
- Participate in staff, team meetings, group, and individual supervision
- Provide direct skill training, case management, supportive counseling and crisis intervention as directed by the individual.
- Facilitate and coordinate, with the individual, access to needed supports, including generic community services.
- Provide support and assistance to individuals necessary for them to develop and maintain relationships with others.
- Provide transportation as appropriate in response to individual schedules and desired activities and/or appointments.
- Work cooperatively/collaboratively with other community service providers and medical professionals to ensure coordination of supports and services.
- Ensure that individuals are treated with respect and dignity at all times and maintain confidentiality regarding their records and other information.
- Ability to communicate and document clearly
- Maintain agency standards of confidentiality
- Perform other job-related responsibilities as assigned

Requirements
Required Qualifications:
Minimum of 1 years’ experience in case management, direct support, or a related human services role.
Knowledge of person-centered planning, developmental disabilities, and community resources.
Strong organizational, documentation, and communication skills.
Ability to work independently while maintaining collaborative relationships with teams, caregivers, and community partners.
CPR/First Aid Certified (or ability to obtain certification within designated timeframe).
Valid driver’s license, KDC vehicle is provided during working hours
