The City Care Housing Stability Advocate s primary role is to work on a collaborative team of Housing Navigators embedded at the Diversion Hub to provide Home visits and wrap-around support services to individuals who have recently transitioned out of an experience of homelessness. The Housing Stability Advocate will help these individuals set goals and objectives within a psychosocial rehabilitation framework that allow for successful housing stabilization and retention.
