Job Type: Part-time, with an opportunity to grow into full-time. A flexible schedule built around low patient volume and generous time for appointments.
Work Setting: Structured in-person, outpatient schedule. No hospital call.
Practice Overview & Philosophy
Cary S. Gunther, MD, PhD, PLLC is an independent boutique practice on the Upper East Side of Manhattan dedicated to unhurried, deeply personalized care for individuals with neurocognitive disorders. Dr. Gunther is triple board-certified in neurology, psychiatry and brain injury medicine.
This practice operates outside the constraints of corporate, insurance-driven, high-volume medicine and provides evidence-based treatment for complex cognitive, mood and behavioral conditions. This is a cash pay practice, and we do not bill insurance. We believe that exceptional care requires generous visit times and genuine partnership with patients and their families.
Position Overview
Patients living with cognitive decline have unique needs that transcend the bounds of neurology and psychiatry and are not met by the assembly-line approach that managed care attempts to dictate. We are seeking a nurse practitioner with experience in geriatrics to collaborate with Dr. Gunther in addressing these distinctive challenges.
Clinical Responsibilities
Longitudinal Disease & Symptom Monitoring: Track management plans for patients navigating Alzheimer's disease, Parkinson's disease, behavioral disturbances and complex medical comorbidities.
Home Safety Visits: Conduct assessments of medication management, falls risk and other aspects of safety in patient homes.
Family & Caregiver Collaboration: Participate in detailed family conferences to navigate disease progression, care transitions and family support throughout the course of neurodegenerative illness.
Care Collaboration: Work in a collegial, thoughtful environment to discuss management strategies and challenging cases.
Location & Transit Access
Address: 955 Lexington Avenue (Upper East Side, Manhattan)
Transit: Conveniently located near the 6 train (68th Street Station), Q train (72nd Street Station) and local bus routes (M98, M101, M102, M103 and M66).
Compensation
$85-$105 / hour, depending on experience.
Starting at two 8-hour days, with opportunities for growth.
Requirements
Education: Master’s (MSN) or Doctorate (DNP) from an CCNE or ACEN-accredited School of Nursing.
Certification: Active Board Certification as an AGPCNP, FNP, or AGACNP.
RN Bedside Experience: Minimum of 2 years of direct patient-care RN experience prior to NP licensure.
Advanced Practice Experience: 1–2 years of APRN experience in neurology, geriatrics, or complex chronic disease management preferred. (New grads considered only with strong bedside RN backgrounds).
Clinical Training Standard: Completion of university-arranged, precepted clinical rotations in outpatient adult or geriatric care. (Minimum 600+ hands-on hours).
Licensure & Prescribing: Active New York State APRN license and active unencumbered federal DEA registration (Schedule II–V).
